As a CCC Independent Distributor, I, __________________ will provide Cascade Candle Company LLC, with a form W-9 so that a form 1099 can be filed with the IRS at the end of each year. I understand that Cascade Candle Company LLC will not withhold any taxes from my commission payments that may be due to the government. I understand that this will be my responsibility. I also understand that I will only contact and sell to individuals and not to any businesses, companies, retail stores, organizations, and non-profit groups. Cascade Candle Company LLC has their own sales reps who sell wholesale to stores. By signing this application, I agree to the terms above and agree that I am an Independent Distributor and no joint venture, partnership, employment, or agency relationship exists between myself and Cascade Candle Company LLC as a result of this agreement or as a result of the use of any Cascade Candle Company products. I understand that Cascade Candle Company reserves the right to approve Independent Distributorship or terminate Independent Distributorship at any time. I also understand that I do not collect any payments for the orders I generate. All payments are to be sent directly to Cascade Candle Company by each customer. Each order form needs to be signed by each customer and the CCC Independent Distributor, or the order will not be processed. It is the CCC Independent Distributor's responsibility to submit orders to Cascade Candle Company for processing, and the customer's responsibility to send in payment for the order. The top page of the order form is to be sent to Cascade Candle Company, the 2nd page is to be kept by the CCC Independent Distributor, and the 3rd page is to remain with the customer as a receipt. Commissions are paid on a monthly basis. Each month is a calendar month, from the 1st day to the last day of each month. In order to be paid their commission on or before the 15th of each month, the Independent Distributor must complete a Commission Request Form and send it to Cascade Candle Company by the 5th of each month. Commissions are based on paid orders within each month, not calculated on submitted orders. Checks for commission payments will be sent out in the US mail on or before the 15th of each month. Commissions are not paid on Shipping and handling charges. No commission payments will be sent until we have a completed CCC Independent Distributor application and a completed W-9 form on file.
